Accepted Documents

Please note: Your proof of address must clearly indicate your current physical address (a mailing address cannot be accepted).
	Utility bills Utility statement of account, hookup, or work order dated within 60 days of the application. Example utilities include cable, natural gas, electric, water, and landline phone (cell phone service cannot be used for address verification).
	Lease or rental agreements Current renters insurance documents, signed rental agreement from property management company, or signed statement from property owner/manager.
	Government/DMV documents Oregon or Washington voter registration card; DMV title, registration, or mail, Oregon DMV address sticker with residential address, or documentation from the Oregon DMV2U website.
	Homeowners insurance documents Policy documents for homeowners insurance.
	Employment & income documents Most recent paystub or benefit award letter (Social Security, VA, unemployment). Paystub or benefit letter must be no more than one month old.
	Tax documents Property tax statements from the current tax year, W-2 tax form, or 1098 or 1099 tax form (no more than one month old).
	Mortgage documents Current statement on a primary residence or mortgage closing documents (signed within the past 6 months).
	Student records Student ID card, student transcripts or other institutional form showing enrollment for the current school year. (Please note: Proof of enrollment cannot be used for address verification.)
